Homeschooling and Exemption Forms
For Homeschooling please fill out the Notice of Intent and the Withdrawal Form.
If your child is between the ages of 7 and 16 and is going to be homeschooled, please complete and submit the Notice of Intent Form.
If you are removing your child from the Shelton Public School System to homeschool, it is very important that you formally withdraw them from the school system using the form listed below.
If your child is 5 or 6 years of age please complete and submit the “Acknowledgement of Option to Exempt Attendance” Form; the “Notice of Intent Form” is not needed.
